SEATTLE (AP) -- The Chicago White Sox requested waivers Wednesday for Bartolo Colon, ending the right-hander's strange second stint with the club.

Colon had offseason surgery to clean up some bone chips in his elbow and agreed to a $1 million, one-year contract with the White Sox in January. But the 2005 AL Cy Young Award winner had more injury problems with Chicago and team officials said he couldn't be located several times during rehab assignments.

White Sox manager Ozzie Guillen also was disappointed that Colon didn't fully participate with the club's rehab plan for him.

"One day he shows up, the next day he doesn't," Guillen said. "He was supposed to do this and that, but he was never with the program."

Colon went 3-6 with a 4.19 ERA in 12 starts this season, missing time with left knee inflammation and elbow soreness. The 36-year-old pitcher was placed on the disabled list for the second time this season in July and hasn't been with the team since.

"When Colon went down for the last time, I knew for a fact he would not be back here," Guillen said before Chicago faced the Seattle Mariners. "At least this year. He couldn't go through the rehab assignment."

Colon was limited to seven starts with Boston last season because of back stiffness that landed him on the disabled list for three months. He wound up going 4-2 with a 3.92 ERA in his lone season with the Red Sox, who suspended him late in the season without pay after he went home to the Dominican Republic and decided not to return. He was unhappy about the prospect of pitching out of the bullpen.

Colon is 153-103 with a 4.10 ERA in 13 seasons with five different clubs. He went 21-8 with a 3.48 ERA when he won the Cy Young Award with the Angels.

"Hopefully he'll be healthy for next year," Guillen said. "I wish him the best."
